What should I bring to my first appointment?

Please be sure to bring a copy of your current insurance card as well as a photo ID. Please print off the patient forms listed below and have them filled out before your appointment.

Where do I park my car?

There is parking available in the gallery office building parking ramp attached to the building. The entrance to the ramp is on Exchange St. They accept cash as well as credit cards. We do not validate parking. There is meter parking around the gallery building as well.

What is going to happen at my first appointment?

The medical assistant will bring you back to check all your vitals: weight, height, blood pressure, and pulse. Depending on your reason of visit, other lab work may be done such as blood draw, ECG, spirometry, etc. The doctor should see you shortly after. Any additional testing ordered by the doctor will be done after the appointment.
